Parish-Wide Discipleship Survey

The parishes in our diocese are participating in a survey about discipleship conducted by the Catholic Leadership Institute. Please help us by clicking the button below and completing the online survey that will take you approximately 10-15 minutes to finish.

The survey will ask you to reflect on your own spiritual growth and provide feedback on our parish’s efforts to help you grow. All responses will be confidential.

We are trying to get the highest response rate possible as we plan for the future!  So, if you prefer a paper copy, they will be available at the Basilica entrances or you can download/print here.  Please return completed surveys to the Rectory Office (767 Ridge Road, Lackawanna 14218).
